Preschool Special Education Referral Process
If your child is between the ages of three and five years old and you suspect he/she may have a disability, please contact us at (209) 933-7345 to begin the referral process.
The 5 Steps

Initiate Referral
Call our office to share your concerns. Staff will complete the referral form with you.

Consultation Scheduled
A phone or in-person consultation/assessment will be scheduled with the Preschool Assessment Team.

Provide Documentation
Submit required documents including proof of residency, birth certificate, parent ID, and any relevant reports.

Evaluation
Parent consent is required before evaluations begin. An Assessment Plan must be signed.

IEP Meeting
An IEP meeting will determine eligibility and services for your child.
Timeline Overview
Referral → Consent
15 calendar days to contact parent and schedule social history. Consent within 30 calendar days.
Summer referrals receive written confirmation and follow-up when school resumes.
Consent → Evaluations
60 calendar days to complete evaluations after parent consent is received.
Consent → Placement
60 school days to complete evaluations, hold IEP meeting, and offer Least Restrictive Environment (LRE).
