Preschool Special Education Referral Process
If your child is between the ages of three and five years old and you suspect he/she may have a disability, please contact us at (209) 933-7345 to begin the referral process for consultation and evaluation.
The 5 Steps of the Referral Process
- Initiate the Referral: Call our office to share your concerns. SUSD staff will complete the referral form with you.
View PAT Referral Form - Consultation Scheduled: A phone consultation or in-person consultation/assessment will be scheduled with the Preschool Assessment Team.
- Provide Documentation: Parents/Guardians must provide required documentation including:
- Proof of residency within SUSD
- Birth certificate
- Parent ID or driver’s license
- Any educational or psychological reports
- Evaluation: Parent consent is required before evaluations can begin. Parents must sign an Assessment Plan.
- IEP Meeting: An IEP meeting will be held to determine eligibility and services.
Understanding the Timeline
The school has 15 calendar days from receiving a referral to contact the parent and schedule a social history. Consent should be obtained within 30 calendar days.
If a referral is received during summer, a confirmation letter will be sent and follow-up will occur once school resumes.
The district has 60 calendar days from the date of parent consent to complete special education evaluations.
The district has 60 school days from consent to complete evaluations, hold the IEP meeting, and offer placement in the Least Restrictive Environment (LRE).
