- School Staff may obtain a TB Risk Assessment by
- Calling Health Services at 209-933-7060 for an appointment (in person or virtual via computer with a camera)
- Via their Primary Care Provider
- Via their site school nurse (if available)

- New Hires may obtain a TB Risk Assessment by
- Calling Health Services at (209) 933-7060 to schedule an appointment (typically offered twice a month)
- Walk-ins are provided only once a month – please call to confirm this date
- Via their Primary Care Provider
- New Hire (SUSD Staff / Table Community / YMCA) should bring a verification email / letter that has been received from Human Resources as to being accepted as a new SUSD employee. This can be shown to the Health Services staff on your phone or you can bring the printed document.
